VIII. ADDITIONAL INFORMATION EXPLANATIONS. (lndicateparto/ReporL) All interest paid the Penn Mutual Life Insurance policy reported Part VII had been used pay premiums the policy. The policy was allowed lapse calendar 2008 when the interest income could longer sustain the premium obligations. The date the lapse approximated. February 2008, filed application for fixed retirement benefits retired member the New York State Employees Retirement System. The application 
was approved the ordinary course and receipt those benefits reported Parts and III-A. Under New York law, any interest had contributions made retirement account reported Part VII was extinguished when the retirement system authorized the payment fixed retirement benefits. The date
