Health Retirement Plan. The filing instructions issued January 2006, pages 43-44 now state that [a]ssets held tax-deferred retirement pension accounts, 
including 40l(k), 403(b) qualify common trust funds, and [t]herefore, the filer not required provide any details about the individual assets held 
those plans, but only required list the name the specific plan Column Accordingly, lines 5-11 and lines and the Report for the year 2004 have been deleted they longer meet the reporting requirements.
