Minor provided additional information regarding her rape. Per minor, she was raped while she was being transferred from
immigration shelter. Minor reported. She was apprehended and taken into custody where she was fingerprinted and
photographed. Minor reports after few hours she was taken immigration officials with other minor girls and they
were being transported another shelter when their truck was stopped. Minor reported men red truck separated
from her the other girls and took her remote location where one the men proceeded rape her while the other
watched. Minor reported she believes they were Mexican but only spoke English each other. Minor reported after the
abuse they dropped her off outside the shelter. Minor reported she was taken staff and was taken care while
there. Minor reported she observed immigration officials transporting other children used code enter the facility and
the men the red truck did not, therefore, leaving her outside the shelter.
Clinician called the Florida Abuse hotline and spoke 2:45 who stated report could not taken and

Summary SIR:
Minor was sent his room during school due disruptive behavior. During school break, separate minor noticed this
minor attempting commit suicide with his sweater. Milieu Supervisor immediately opened this minor room door and
sent all other minors down their rooms while County staff called for back up. During this time, Milieu Supervisor
talked this minor, asking him remove the sweater. program staff approached, this minor began tighten his
sweater and tried kick staff. County staff then utilized CPI restraining techniques interrupt the suicide attempt and
then escorted this minor the medical close observation room. Following the use CPI restraining techniques, this minor
made allegation program Clinician stating was punched the face his left cheek County staff
involved the restraint. However, hour later, this minor stated that this County staff accidentally hit him the face
they were trying remove the sweater from around the minor neck during the intervention.
Addendum: followed with investigation regarding allegation. Based all information and accounts, misuse
abuse restraint was issued against minor. also spoke with FFS provide update and information which conclude
investigation and abuse minor was incurred.
