Incident Reports (SIRs) from May 2014 through November 12, 2014, and associated summary reports. are attaching responsive summary reports and look forward further discussion about whether these
materials can satisfy your request. have determined withhold information under the FOIA exemption (b)(6), which protects
information about individuals when disclosure would constitute clearly unwarranted invasion
personal privacy. this case, the withheld information consists personally identifiable information
unaccompanied children and associated private individuals and employees the grantees.
withholding the information, each individual privacy interest was balanced against any public interest
disclosure. each instance where information was withheld, determined that the individual privacy
interests safeguarding their personal information from abuse, fraud and theft outweighed any public
interest disclosure the withheld information and that disclosure the withheld information would

During the course initial assessment, minor disclosed significant history verbal and physical abuse, well forced
labor grandmother. Minor indicates that she has lived the care her grandmother since her mother immigrated
the United States approximately years ago, and that her grandmother has hit her and verbally abused her that entire
time. Minor indicates that her grandmother would hit her with her fists and any objects lying around such belts, sticks,
flat sides knives, cooking spoons, etc. Minor recounts one instance when her grandmother struck her repeatedly the
face, resulting black eyes and bloody nose. Minor indicates that her grandmother would sometimes strike her front other workers the tortilla plant front family members, and would publicly belittle her. Minor also discloses
that her grandmother would refuse pay for minors personal expenses, such necessary clothes school supplies, and
when mother would send money from the fund minor, minor felt that she had hide the money from her
grandmother else she would take away. Minor has indicated that she was forced work compensation for her
grandmothers care, and would often work hour days without pay. Minor indicates that grandmother would also

did not. Client states she then told him she could give him the piece she was chewing. Client reports she placed the piece

Client states turned his face thought that she would kiss him and told her No, don want it, full germs another occasion, client states that
was ending her visitation and decided give him hug before
leaving. Client states that she gave him tight hug and sucked his neck. Client explained that was not hickey
kiss specifically reports being suction made with her lips the left side his neck. Client tried avoid pretending asleep. would also try change conversation when comments she made were
inappropriate, and pretended pain the visitation would terminate. Finally, client mentioned that while class,
ended give him much attention and would want sit next him during lunch time. While sitting

would constantly rub her leg against his. Also, while class, states
would receive letters from her daily basis with compromising content. YCW place was immediately instructed not leave client unattended unsupervised during visitations.

Client requested for Unit Supervisor help him pack and clean his room when felt over whelmed. Staff
were the room doing inventory his personal belonging. When Unit supervisor was cleaning out his desk
drawers noticed glue plastic-wrapped into ball.

picked folded paper and opened and then
noticed white powdery substance.

immediately took the paper and placed one the staff drawers the staff area. Unit Supervisor contacted Chemical
Dependency and was told write SIR. The Program Director was also contacted and suggested take the substance
the Fullerton Police Department.

doing? Client stated was throwing something away. collected the paper and the glue from the staff drawer and put
them into zip lock bag and took the police station and spoke Officer Park. The officer took the substance get tested when returned stated there was nothing the paper any type residue was not able conduct any
type test. Officer Park said for future reference not transport any type suspicious substance, contact the police
and isolate the evidence and not let anyone touch until the police arrive. When returned the unit, checked the
trash can and noticed there was white powder the trashcan. immediately notified Unit Supervisor
and Assistant

whom notified Program Director then got the approval program director contact
the police department and police officer was immediately dispatched. The police arrived the unit and spoke Unit
Supervisor
Another officer arrived moments later and tested the substance. Officer Phu said there was not
enough the substance for tested. Both officers spoke client the kitchen and then proceed search
his room. The police officer did not find anything else. Officer Phu mentioned that client
had stated that was pill
for headache and that received from another client and crushed the pill.

out the home. After the months did tell his uncle that wanted paid. stated that his uncle then gave him per week. After three weeks
told him that this was too little, and stated that his uncle said, giving you
this money because want to. not for the work that you are doing. You are working here because you are living
house. When
was asked this was something agreed any point him living there stated that
wasnt and that his father was still sending his aunt money every month. When asked ever had day off, stated
that his aunt would able occasionally convince her husband have
stay home.

Youth disclosed that she was sexually assaulted unknown male home country, Salvador, and currently
months pregnant. Youth stated Dec. 20, 2013 she was victim sexual assault unknown male who entered their
home when she was alone. Youth stated she only recalls male coming her with rag his hand and does not
remember anything else, but him wearing black. Youths brother found her with blanket very dazed and confused. She
was then taken the hospital where she was medically reviewed for days. The doctor shared with her that she had been victim rape. The youth stated she was upset but couldnt recall anything. The doctor advised her that she could become
pregnant due the sexual encounter. Youth stated January 11, 2014 she found out she was pregnant. Youth added this
was her first sexual encounter, she denied being sexually active prior the incident. Youth has very positive attitude
toward this pregnancy, due the circumstance. She stated that the situation was unfortunate but God would not give
child life without purpose and although hurts her know how came about she tries see the way God and her faith
allows her too. Youth stated her brother and mother have been encouraging and supportive throughout the process her
understanding. Youth four months pregnant and expecting boy. Youth stated the perpetrator has never come back and
she was not being threatened anyone. Youth stated she migrated the with her mother.

that during her journey from Salvador Mexico, her guide left her warehouse
Mexico with his boss, narco, and group other guides. Client reports this occurred about month ago and
that she was the warehouse for week and half. Client does not know why she was left there and asked continue with
her group, but her guide said that she had say. Client stated the warehouse was used location for guides and narcos
drink, smoke, and meet with the boss. While client was there, she was not given food phone calls and was unable
sleep due being afraid. Twice she went the nearby river used for crossing immigrants and was told observe she
could someday guide. When client was not the river, she spent her time inside crying hitting her head against
wall hoping kill herself. Client reported that her guide boss came into the warehouse often and while was there
kissed her, groped/bit her chest, pinched her legs. Client has several small bruises her legs that are still visible and
showed them clinician. Client stated the guide boss also put gun her head and told her stop crying that
would kill her. Client reported they also threatened kill her entire family, but denied that they knew any information
about her family Salvador. One day, her guide boss came the warehouse and had two men hold her hands back
while raped her. Client states she was raped three times that day the guide boss and was told that she was his
mujer (woman). Client stated the guide boss the only one who touched/raped her. After week and half the
warehouse, client was told that she could leave. Client crossed the river alone and shortly after, immigration helicopter
apprehended her and took her immigration holding facility. Client was noted tearful and sad when she
recounted the story. Client said that when she left alone, she starts feeling afraid and thinking the man who raped her
and wants kill herself she can with God. Client said that she wanted kill herself, she could use any available
object asphyxiate herself. Client denies any current intent kill herself because she not alone and feels supported and
safe the shelter environment. Client reports one previous suicide attempt year ago; client took blade and cut her
wrist due being bullied her school. Client reports severe sleep disturbances, stating that she unable sleep because
she thinks the man who raped her that when she does fall asleep, she has nightmares about what happened the
warehouse. Client reports appetite disturbances and states that last night was the first time she ate almost weeks.
Client finished half her breakfast 6/14/14 while speaking with clinician and finished most her lunch 6/14/14
while speaking with clinician. Client reports having atackes nervios (anxiety attacks) well, with the last attack
occurring last night the shelter. Client describes these attacks several minutes when she cannot breathe, begins

she also experienced flashbacks while the immigration facility and thought that she was back the warehouse with the

back cottage. One SAPD unit went check perimeter Walmart for resident; second unit followed Seton Home retrieve
information for Missing Persons report. SAPD was given description
her full name, A#, DOB,
POB, and basic med info. Law enforcement was notified that she under ORR care. Program director was contacted and
case manager, Programs, ICE, FFS, and were all informed incident. Hotline call CPS and SIR were submitted

would have package marijuana leaves and cocaine milk from the cocaine flower during the night. She stated she was

families were threatened that the minors would continue working. Minor always told the cartel members that was
abandoned and that had family, order protect his family. Minor reports having been abused these people
the following ways: They tied him various occasions for week time. They hit him, pushed him down and
broke his tooth, and beat him multiple occasions which left him swollen, bruised and scarred his backside. Minor
reported that didnt the hospital after his beatings even though was badly hurt because there are cartels
informants working there. reported that simply sat bathtub cold water feel better. reported that his last
beating this type was about two months ago (April 2014) which received hits the back side large
wooden boat paddle with holes that they could swing faster and harder. Minor was seen doctor Madison
School Based Health Center 6/9/14 and was found that minor does have bruising and scars his backside. stated
that one point the cartel locked him house where people stayed while waiting cross the border and made him
clean the house and wash clothes. stated that the house was guarded and that was rarely allowed leave the house
and even when left the house that they had people all over the city watching and could not get away. Minor
appeared very anxious and was emotional. had some trouble answering questions due being pressured and anxious,
but was cooperative and expressed gratitude for being listened to. reports being unsure who can trust, but desperate
for help. Minor reports that while enslaved the cartel thought drinking bleach that used clean the house
where they had him, but did not because had dream that told him not give and did not want hurt his
family. denies any current suicidal ideas, plans intentions. Minor reports that while still Mexico that had some
